with how Mount Olive Township and the Mount Olive School
Board schedule our Facilities and Fields. Please fill out
and return the following documents.
*Your events will not be approved until these forms
are received by the Recreation Department*
The following forms are for Facility
and Field Requests:
Field
and Facility Permit Application - Required by
the Township (adobe acrobat)
Facility
Usage Form - Required by the Board of Education

Muncipal Building and
Senior Center request must be made thru Schooldude.

Proof of insurance is required , additional paperwork
will be issued upon review of request.
Field
Rental Fee - Required for groups not sponsored
by the Township or Board of Education (adobe acrobat)
Turkey
Brook Gazebo Rental-Required for groups not sponsored
by the Township or Board of Education (adobe acrobat)

them to the Recreation Department or mail them to:
