Welcome to Scheduling Information page. The following information is provided to assist you with how Mount Olive Township and the Mount Olive School Board schedule our Facilities and Fields. Please fill out and return the following documents.


*Your events will not be approved until these forms
are received by the Recreation Department*

The following forms are for Facility and Field Requests:

Field and Facility Permit Application - Required by the Township (adobe acrobat)
Facility Usage Form - Required by the Board of Education (adobe acrobat)

The following form is for Field Requests only:

Field Rental Fee - Required for groups not sponsored by the Township or Board of Education (adobe acrobat)

After completing these forms please return them to the Recreation Department or mail them to:

Once your forms are completed please use the Links and Directions provided below to make your request. *You must use our on-line scheduling process in order to make your request.*
